As a disclaimer, I don't use floquet ports, but the setup looks correct to me as far as I know (hfss 13). For the master/slave boundaries, you probably just want to use the specified phase angle rather than theta/phi for square structures. I have a problem calculating the phase of the reflection coefficient for an AMC material. My structure is formed by a silicon substrate and 2 oxides on top of it.

in hfss or any FEM solver, you are the only one responsible for the design! and believe me, it is simple. i try again:
0- i assume 2D, xy plane.
1- you have a freq range with their k values and teta
2- put in the source expression, the x component of your incidence kx=k*cos

in my design, i used wave port,solution type:driven modal,i had a radiation boundary.
After selecting the desired part of design, you should this steps:
right click-plot fields-select E or H or...
