For cst did you try a multi-pin port? I think this is a special case of the waveguide port. You define the center conductor as signal 1, and the shields to be signal -1. I think you reach this feature in the Waveguide Port definition window; there is a button called "Define Pins" at the bottom of the window that then lets you click on the edges
I my opinion the with and height of cpw depends upon the choice of frequency rage, higher the frequency lower the width and height of port. A good idea is to adjust the port dimension near 50ohm.

Hi, I had a quick look at your structure. The Vias in your model are Vacuum. Are you sure this is correct? Furthermore the Ports 2 & 3 are directly located at a GND bridge. Because the GND bridges are not connected with the main GND (vias problem) the port mode solvers sees some "floating" metal plates right at the port. Please change this. Fina
just draw your cpw antenna, and assign a waveport as an excitation. The wave port should be about 3*h above and below the substrate, with h being the substrate height.
