the word direct in "Direct Form II" means the coefficients in H(z) is directly used in the signal flow chart.
IC 14 appears to be a chip labelled 'OSC'. The only terminals are the supply pins, and clock out. I don't believe it's a particular IC, but rather more like a flow-chart icon meaning 'clock generator of whatever sort you wish to use.' As to the MHz, it also appears to be unspecified. It's probably meant for you to experiment.
I think the best you can do is to disassemble to asm code then use something like Visustin or similar to convert code to flow chart. Not sure if that particular one would work for your code but supports MASM.
